- Complete the Life/Beneficiary Electronic Enrollment Form AND
- Complete the Evidence of Insurability Form
You may enroll in Life Insurance anytime during the plan year.
South Dakota State Basic Life Coverage:
A Basic Life Coverage of $25,000 is provided to Benefit Eligible Employees by the State of South Dakota.
South Dakota State Supplemental Life Coverage:
The Employee may choose Supplemental Life Coverage equal to two, three, four, or five
times annual earnings (rounded to the next highest multiple of $1,000, but in no event
shall the amount of coverage exceed $400,0000.)

South Dakota State Dependent Life Coverage:
Employees who are covered under Supplemental Life Coverage may elect $10,000 Dependent Life Coverage.

You must be enrolled in Supplemental Life Coverage-see above on how to enroll.
South Dakota State Accidental Death and Dismemberment (AD&D) Coverage :
Employees who are covered under Supplemental Life Coverage may elect AD&D Coverage.
To enroll for coverage under this Plan:
AD&D Coverage must equal the Supplemental Life Coverage selected.
If you enroll in AD&D, your AD&D Coverage will apply to Dependent Life Coverage as well, if applicable.

ING Life Coverage:
Employees can purchase more life coverage with ING, underwritten by ReliaStar Life Insurance Company.
Note: ING / ReliaStar Life Coverage is not the same as the South Dakota State Life Coverage, and is available for Central Payroll employees (not Board of Regent employees).
