   • Increase Dependent Health Plan Contribution Rates
   • Increase in Dental and Vision Contribution Rates
   • Remove all copayments from the $500 Deductible and $1,000 Deductible Plan except:
     • $250 Emergency Room
   • CVS Caremark replace ESI as the Pharmacy Benefit Manager
